RN Case Manager - Syracuse, NY

The RN Case Manager is responsible for providing case management to patients  through a coordinated process of assessment, planning, implementation and advocacy for options and services to meet a patient's health needs. Works to develop and implement a comprehensive care plan for each patient's identified needs. 
1.    Conducts home / facility visits for comprehensive health assessments / reassessments to identify health and environmental needs. 
2.    Collaborates to develop their individualized short and long-term goals. 

RN Pediatrics, OB/GYN or Adult Medicine - Syracuse, NY area

Provide direct nursing care and education to meet the needs of patient population and assist other nursing staff members and department providers in the provision of quality care. Perform administrative duties to maximize department efficiency. Ensure positive customer experience.

Licensed Practical Nurse - Syracuse, NY area

Provide direct nursing care and patient education to meet the needs of client population; assist other nursing staff members and department providers in the provision of quality care.

•    2+ years experience
•    Current NYS license and registration in good standing as an LPN
•    Degree from an accredited school of Nursing
•    Current with CPR certification and Infection Control Certification
 

LPN Team Leader - Syracuse, NY

This position supervises the staff and manages all nursing activities for patients in the assigned unit
